With this plan, you eliminate monthly deductibles and only pay when you have dental work done. There is no hassle and no waiting periods. Simply pay one low annual fee and you will receive a discount on your dental work, as well as free x-rays, exams, and cleanings.

+Add additional family members for only $349 each per year

•  A complete initial dental exam (Normal price $96.00)
• Periodic exam (Normal price $65.00)
• Up to two cleanings per year (codes 1110, 1120). Regular cleanings are for patients with healthy gum tissue (Normal price $210.00).
• One Full Mouth Set of X-Rays During the Initial Exam  (bitewings-codes 0274 or 0272 and periapicals-codes 0220 or 0230). (Normal price $150.00)
• One panoramic radiography taken at first recall visit (code 0330). (Normal price $119.00)
• Up to two fluoride treatments for childern 12 and under. (Normal price $160.00)

• Regular cleanings are for patients with healthy gum tissue.
• 10% off treatment must be completed within the calendar year of your plan and it excludes orthodonics
• All services must be scheduled and completed before the plan year ends.
• You will receive 10% off any additional exams and x-rays needed for diagnosis of additional treatments.
• Additional family members can be added at any time and the plan end date is the same for all family members, regardless of when the plan begins.
• Your Membership Plan may not be combined with other offers.
• Your plan is not transferrable, no refunds, no exceptions.
• For your convenience, memberships automatically renew annually.
• Your Membership Plan cannot be combined with dental insurance.
