Welcome to Cary Dental Arts
We are delighted to welcome you to our practice and are pleased that you chose us to serve your dental needs.
Please complete below forms ahead of time if possible, and arrive 10 minutes early to your 1st appointment, allowing time for our staff to review your information. If you wish to complete these at our office, please arrive at least 20 minutes early. Of course, if you have any questions just give us a call at 919.297.2701 or ask us upon arriving at our office.
Your First Appointment
Our entire dental team look forward meeting you and will make you feel comfortable from the moment you enter our beautiful ultra modern dental office.
Each patient that we have the privilege to serve will receive a thorough examination. This examination includes the necessary radiographs and a complete oral examination as required to make an accurate analysis of your oral condition. A diagnosis, prognosis and written estimate of the treatment fees involved will be provided.
Our office hours are from 9:00 AM to 5:30 PM on Monday, Wednesday, Thursday and Friday, 9:00 AM to 3:00 PM on Saturday (closed on Sunday & Tuesday). An answering machine will record your call and take messages when the office is closed. For your convenience, appointments at other times may be arranged upon request. Our receptionist is responsible for the schedule of appointments. She will reserve a time that is most convenient for you. Except for emergency cases, you may expect us to be on time, and we would appreciate the same courtesy.
Please make every effort to be on time for your appointment! In case of an emergency, and you must cancel your appointment, please advise us at least 24 hours in advance so that no charge will be made, and we can arrange to give your reserved time to someone else.
